Creating person-centered care plans seems so simple, intuitive even. ‘Person-centered’ has become a universal term yet we describe it differently and struggle with how to operationalize it. In this session, we’ll explore the qualities of a person-centered care plan and most importantly, how to highlight a strengths approach that blends the medical model and a trauma-informed care model with the main focus to improve the individual’s quality of life. By incorporating multiple examples from actual care plans, complicated areas such as care planning indicators of distress will be simplified, and staff empowered to affect positive influence in residents’ lives.Expand summary
